陰茎持続勃起症に対する陰茎海綿体大伏在静脈吻合術

CAVERNO-SAPHENOUS SHUNT IN THE TREATMENT OF PRIAPISM

　Priapism陰茎持続勃起症とはギリシャ神話に出てくる受精の象微である神Priapusから名付けられたという^1）。これは性的欲望がないにもかかわらず陰茎が持続的に勃起し，一般に疼痛を伴つている。持続勃起に対する治療法の原則は陰茎海綿体内の血液を排除し，陰茎を弛緩させることである。これにより，疼痛から解放され，また，ひきつづきおこる勃起不全を避けることができる。現在までに種々の手術法が考案されているが，いかなる方法であつても海綿体内の血液を排除できるのであればよい手術法といえる。1964年Grayhackらにより行なわれた陰茎海綿体大伏在静脈吻合術^2）は内外においてすでに多くの報告がみられている。われわれは最近経験した陰茎持続勃起症6例に対しGrayhack法を行なつた。そのうち2例に対しては術後，自動的陰茎圧迫を試みた。ここにわれわれの成績を報告する。

Six cases with priapism were treated by cavernosaphenous shunt, described by Grayhack and asso-ciates. Manual or automatic penile pressure was applied in order to maintain blood flow in the saphenous shunt following surgical procedure. All of our patients were relieved from the condition of the disease one to three days after the operation. During the follow up period, three cases recovered normal sexual power, one experienced partial return of sexual function, but one could not produce any further erection. One died of acute renal failure during the hospitalization.
