転移を有する小児神経芽細胞腫の手術症例

SURGICAL TREATMENT FOR METASTATIC NEUROBLASTOMA: A CASE REPORT

緒言

　神経芽細胞腫はウイルムス腫瘍とならんで小児の腹部悪性腫瘍の代表的なものでその予後は悪い。小児悪性腫瘍の全国統計では，1969〜1972年の4年間に391例が登録されている^1）。

　われわれは，骨髄および後腹膜リンパ節転移を有する6歳男子の神経芽細胞腫症例に対し，原発巣と考えられた左副腎腫瘍を摘除し，一時的寛解を得たので報告し，転移のある本疾患の手術療法の意義について文献的に考察した。

A 6 years old boy was admitted to Ibaraki Central Hospital for slight fever, pain of bilateral knee joints and left abdomen. He was diagnosed as left adrenal neuroblastoma with metastases to bone marrow and retroperitoneal lymphnodes. Combination of chemotherapy and irradiation showed no effect to the metastatic site. However, after extirpation of primary site, the size of metastases of retro-peritoneal lymphondes decreased markedly. Five months after operation, the size of retroperitoneal and other lymphnodes metastases became large and one year after operation, liver and urinary bladder metastases were noticed.
