下大静脈後尿管の1治験例

SURGICAL CORRECTION OF RETROCAVAL URETER: A CASE REPORT

はじめに

　水腎症は日常診療でたびたび遭遇する疾患で，その原因としては尿路結石，尿路結核および先天性腎盂尿管移行部狭窄などが多い。血管が原因になつているものとしては，異常血管が尿管をしめつけるような走行をしていることがめずらしくないが，さらに稀なものとして下大静脈後尿管（Retrocaval ureter）がある。本症は尿管が下大静脈の後を通る先天性の奇型で右側にあるのが普通である。

　現在，本邦で75例（1969年）の集計があるが，われわれは，本症の1治験例を経験したのでその概略を報告し，若干の文献的考察を行なつた。

A case of retrocaval ureter in a 17 year old male, corrected surgically, was reported. The diagnosis was confirmed by the combination of retrograde pyelogram and vena-cavogram. Ureteroureteral an-astomosis was done followed by nephropexy according to Demings method. Remarkable recovery of right hydronephrosis was confirmed by I.V.P. done 6 months later.

The literature was reviewed and the surgical methods were discussed.
