下大静脈後尿管

RETROCAVAL URETER

Ⅰ．緒言

　本症は比較的稀な下大静脈の発生異常であるが，そのurogramで典型像を呈するものでは今日，その診断は極めて容易であり，最近の報告例では殆んどが術前診断が行なわれている。しかし稀に典型像を呈しない症例がありurogramの注意深い判読が要求される。これについては土屋らの詳細な記述がある。とはいえ今日，下大静脈後尿管における興味は専らその手術方法に向けられているようである。

　筆者は最近相次いで本症の2例を経験したが，1例はurogramで典型像を呈しており術前に診断しえて腎盂尿管吻合術（Harrill）を行ない，他の1例は非典型像で手術時に初めて本症である事が判明した。以下これらの症例について報告し，1966年末までの本邦例59例について観察し特にその治療について考察する。

Retrocaval ureter, a congenital malformation, is most easily diagnosed by its characteristic findingsby urogram and case reports are increasing, but occationally some atypical cases has some difficulty to establish the diagnosis.

Recently, I have experienced 2 cases, one being diagnosed preoperatively from its typical findings and plastic surgery was done by Harrill's method, and the other case was an atypical one discovered during surgery complicated with renal tuberculosis and nephrectomy was performed.

59 cases from the Japanese literature were collected and statistical study and treatment was discussed.
