高Ca血症を伴う腎腺癌症例

RENAL ADENOCARCINOMA ASSOCIATED WITH HYPERCALCEMIA

緒言

　比較的稀ではあるが，腎癌，乳癌，肺癌などの悪性腫瘍の患者で，骨には転移その他の破壊性病変がなく，かつ副甲状腺機能にも異常がないにもかかわらず，著明な高Ca血症を示すものがあることは，かなり以前より注目されており，その検索についても種々の論議が行なわれてきている。

　今回われわれも右の範ちゆうに入る腎腫瘍患者の高Ca血症を経験したので，以下に記録すると共に，本邦の悪性腫瘍に伴つた高Ca血症を渉猟し，かつ高Ca血症の成因などについて若干の文献的考察を加えてみた。

A 55 year-old male visited our clinic with the chief complaint of mass at right flank and edema of both extremities in May 1969, who had been treated for renal disease by a nearby physician for one year. Past history reveals no urinary tract stone, symptoms of digestive tract, intake of vitamin D nor alkaline drug. Anemia was remarkable and an infant-head-sized mass extending 2 finger width below the umbilicus was palpated at the right flank by admission. Blood pressure was 140/90, Blood sedimen-tation rate 145mm at one hour, B.U.N. 16. 2mg/dl.
