乳糜尿に合併した腎盂軟結石の1例

A CASE OF SOFT STONE OF KIDNEYPELVIS COMPLICATED WITH CHYLURIA

緒言

　軟結石とは，チスチン結石およびUrostealithを除く軟かい性状の結石の総称であり，現在まで，フイブリン結石，蛋白結石，細菌結石などが報告されている。通常の硬結石に比して非常に少なく，従来より主たる成分により上記のごとく分類されているが，その内容は多種の有機化合物で，分子量も広範囲にわたつており，統一的見解は，いまだ示されていない。また，成因に関係あると考えられる合併症も種々，報告されているが，われわれが今回経験した乳糜尿を伴うものは，本邦では調査した限りにおいて，いまだ報告されていない。

Case is a 57 years old male who visited our clinic with the chief complaint of white cloudy urine and recurrent pyelonephritis. Past history reveals that he had been in the tropical area during the 2nd world war and was affected by malaria. Various examination lead to the diagnosis of chronic pyelonep-hritis due to right renal stone with chyluria from both kidneys. Since chyluria did not respond to any type of conservative treatment, decapsulation of the right kidney was performed and right nephrectomy for multiple renal calculi and recurrent acute pyelonephritis.
