自然腎盂外溢流を契機に診断された右尿管癌

Ureteral carcinoma found by spontaneous peripelvic extravasation：A case report

　症例は75歳，女性。主訴は右背部痛。腹部CTにて右水腎症および腎周囲への尿貯留が疑われた。逆行性腎盂造影にて右尿管癌と診断し，腎尿管全摘除術および膀胱部分切除術を施行した。摘出標本では明らかな破裂部位は認められず，自然腎盂外溢流と診断した。病理診断ではurothelial carcinoma，G2＞G3，pT3であった。自然腎盂外溢流の原因として，悪性腫瘍の合併にも注意する必要があると思われた。

A case of ureteral carcinoma found by spontaneous peripelvic extravasation is reported. A 75-year-old woman presented with right back pain. Abdominal computed tomographic (CT) scan showed right hydronephrosis with extravasation of urine around renal pelvis. Retrograde pyelogram (RP) was performed and showed the filling defect in the right lower ureter. Under diagnosis of ureteral tumor, right nephroureterectomy was performed. Histological finding revealed urothelial carcinoma, G2＞G3, pT3, INFβ, ew0, ly1, v0. After operation, she was free from recurrence during 2 months.（Rinsho Hinyokika 62：61-63, 2008）
