腎静脈内腫瘍血栓を伴った腎平滑筋肉腫

A case of renal leiomyosarcoma with renal vein thrombosis

53歳，女性。左側腹部痛および腫瘤触知を主訴に当院を受診した。画像診断にて左腎静脈内腫瘍血栓を伴う腎細胞癌と診断し，経腹膜的左腎摘出術を施行した。病理診断の結果は腎平滑筋肉腫であった。術後14か月現在再発転移は認めていない。腎平滑筋肉腫は比較的稀な疾患であり，自験例は本邦101例目であった。また腎静脈内腫瘍血栓を伴った症例は自験例で5例目であった。

A 53-years-old female was hospitalized with complaints of left frank mass and pain. CT scan revealed a left renal tumor with renal vein thrombosis. Transperitoneal radical left nephrectomy was performed. Histological diagnosis was renal leiomyosarcoma with renal vein thrombosis. Without adjuvant therapy,she is free of symptoms with no evidence of local recurrence or distant metastases 14months postoperatively. This is the 101st case of renal leiomyosarcoma reported in Japan,and 5th case of renal leiomyosarcoma with renal vein thrombosis.
