ON HYALINOSIS CUTIS ET MUCOSAE

Ⅰ．はじめに

　Hyalinosis cutis et mucosaeを最初に記載したのは，1907年Baselの耳鼻科教授Siebenmann^1）である。観察例は19歳女子で，"UeberMitbeteiligung der Schleimhaut bei allgemeiner Hyperkeratose der Haut"と題した。その皮膚の変化についてはichthyosiforme Erkrankungと記しているものの，その所見は皮膚が粗糙であるのみならず，おろし金様を呈し，針頭大の丘疹がとくに四肢には混在し，眼瞼縁睫毛部には真珠頸飾様配列を示す白色調丘疹が認められ，口唇，舌，咽頭等上気道粘膜には著明な黄白色斑が多発しており，生時より嗄声があつたという。病理組織学的には，背部および上腕の切片には，病変部位が当つていないのか，本症特有の所見が記されていないのに対し，粘膜の標本には血管および粘液腺部にビアリン様物質を認め，その図譜には今日見る典型的組織像が画かれている。

　その後観察例の追加が相次ぎ，既に150例以上の報告が数えられる^2）3）。しかるに本邦においては，片岡ら^4），伊崎ら^5）の症例を見るにすぎず，筆者の観察は第3例に該当すると思われる。それで，軽症ではあるがこれを記載し，同時に文献を主としてその本態を勘案し，Hyalinosis cutiset mucosaeの名称を採つてここに報告したい。

　A case, 24 year old man, with hyalinosis cutis et mucosae is reported. Although his skin lesions were relatively moderate, the serum protein analysis showed marked low albumin and high α₂, β and γ globulin levels, and a slightly high serum mucoprotein level. The content of the protein-bound-disaccharide was within a normal range, and the electrophoretic pattern, however, showed high α₁ globulin and low α₂ and γ globulin.

　Histopathology of the skin specimens showed no presence of the fat droplet. This patient appeared to have some inherited abnormalities in the carbohydrate and protein metabolisms and in addition to these, it seems that some local changes may have occurred in the skin and been followed by the deposition of hyalin material.

　In discussing the literatures on hyalinosis cutis et mucosae, the clinical pictures, inheritance, autopsy findings, and electronmicroscopic studies of this disease were described. The term "hyalinosis cutis et mucosae" is thought to be quite suitable and useful.
