滲出性中耳炎に対するCO2レーザーによる鼓膜切開術の経験

CO2 Laser-Assisted Myringotomy for Otitis Media with Effusion

　はじめに

　滲出性中耳炎の治療には保存的治療，鼓膜切開術，中耳換気チューブ挿人術などがある。保存的治療を行い改善が認められない症例に対して，メスによる鼓膜切開術を行うことがある。しかし，メスによる切開では穿孔は数日のうちに閉鎖し，再貯留を認めることがある。

　今回，CO₂レーザーによる鼓膜切開術を行い，平均19.1日間の開存期間を得ることができた。その後，再貯留した症例にのみ中耳換気チューブ挿入術を行った。中耳換気チューブ挿入術適応の判断に，CO₂レーザーによる鼓膜切開術は有用な手技と考えられたので報告する。

CO₂ Laser-assisted myringotomy for otitis media with effusion was performed in 83 patients (124 ears).

The average closure time was 19.1 days. 59.7% of all patients did not require a second treatment within 3 months after closure.

There were no postoperative complications such as permanent perforation, otorrhea and thinning of tym-panic membrane.

CO₂ Laser-assisted myringotomy seemed to be an effective treatment before insertion of ventilation tube was indicated.
