A 4-year-old girl with a brainstem tumor was diag-nosed at other clinic as Bell's palsy.

Right mild sensorineural hearing loss and nystagmustoward the left strongly suggested a central lesion for the cause of right facial palsy.

Space occupying lesion was revealed in brainstem by MRI.

Unfortunately surgical removal and postoperative irra-diation were not effective and she died in one year later.

Histopathological diagnosis was pilocytic astrocytoma.

It is necessary to rule out neoplastic origin before making a diagnosis of Bell's palsy in small children.
