めまいを初発症状とした椎骨動脈解離性動脈瘤の1症例

Vertigo due to Vertebral Artery Dissection:A Case Report

　はじめに

　椎骨動脈解離性動脈瘤は後頭蓋窩くも膜下出血の原因の28％を占めるといわれ^1），最近脳神経外科領域で注目されている疾患である。また，前駆症状としてめまい，後頸部痛を呈することが多いとされ^1,2），脳神経外科医のみならず神経耳科医もその存在は知っておくべきである。

　今回われわれは，めまいを主訴で横浜赤十字病院を受診した椎骨動脈解離性動脈瘤症例を経験したので，その概要に若干の文献的考察を加えて報告する。

A 46-year-old male presented with central positional vertigo and infarction in the bilateral lower cerebellar hemispheres due to the right verte-bral artery dissection. MRA with 3 D-phase con-trast showed abnormal intensity in the right verte-bral artery. Spontaneous nystagmus of sitting posi-tion toward the left side was found. The patient complained of positional vertigo and showed positional right-beating nystagmus when his head was tilted toward the left under Frenzel's glasses. It was assumed that this positional nystagmus was caused by the lesion in the vestibulocerebellar sys-tem.
