A 46-year-old male presented with central positional vertigo and infarction in the bilateral lower cerebellar hemispheres due to the right verte-bral artery dissection. MRA with 3 D-phase con-trast showed abnormal intensity in the right verte-bral artery. Spontaneous nystagmus of sitting posi-tion toward the left side was found. The patient complained of positional vertigo and showed positional right-beating nystagmus when his head was tilted toward the left under Frenzel's glasses. It was assumed that this positional nystagmus was caused by the lesion in the vestibulocerebellar sys-tem.
