Multiple cranial nerve dysfunction was developed 8 day after uncomplicated neurovascular decom-pression for hemifacial spasm. Ipsilateral multiple cranial nerve dysfunction including facial hypes-thesia, incomplete facial palsy, loss of gustatory sensation, ear fullness, incomplete recurrent nerve palsy, and dysphagia were identified, but there were no signs of auricular herpes zoster. Mechanical minor trauma would initiate reactivation of varicel-la-zoster virus in the geniculate ganglion of the facial nerve in this case. Recovery of all cranial nerve palsies was obtained by conservative treat-ment within one or two months.
