労作時呼吸困難と発声困難を主訴とした陳旧性披裂軟骨脱臼の治験例

A Case of Long-Standing Arytenoid Dislocation caused by a Traffic Accident

　はじめに

　喉頭外傷における披裂軟骨脱臼は稀な疾患であるとされてきた。その発生機序としては交通事故などの体外からの力で発生する外損傷性のものと，気管内挿管や消化管内視鏡検査などの医療行為時に発生する内損傷性の喉頭外傷とに大きく分けられる。特に外損傷性の場合，輪状披裂関節が喉頭の後方に位置し可動性も大きく弾力に富んだ関節包であるため，単独で生じたという報告はきわめて少ない^1〜3）。今回われわれは，幼少時期の交通外傷が原因と考えられる披裂軟骨脱臼による嗄声と労作時の呼吸困難を訴えた症例を経験し，その音声改善に苦慮したので若干の文献的考察を加え報告する。

A 33-year-old man presented with hoarseness and dyspnea on exercise.

Laryngeal fiberscopy revealed that granulation-like protrusion over the right vocal fold. CT exami-nation showed antero-medial dislocation of the arytenoid cartilage.

Endoscopic laser arytenoidectomy improved the airway to reduce dyspnea on exercise. But hoarse-ness was difficult to control because of tight fixa-tion and scar formation of the vocal fold mucosa.

Thyroplasty type I was tried to improve his voice without apparent effect.
