椎骨脳底動脈循環不全によると考えられた感音難聴

Sensorineural Hearing Disturbance due to Vertebro-basilar Artery Insufficiency

　はじめに

　感音難聴の原因として遺伝的素因，ウイルス，音響，薬物，老化など多くのものが知られている。しかし，一方では原因のよくわからない感音難聴の症例に遭遇することもままあり，特に進行性であったり変動する場合，診断や治療に苦慮する。今回それらの難聴の原因の1つに内耳への血流障害があるのではないかと考え，臨床的検討を行ったので報告する。

Seven cases with progressive hearing loss were studied clinically, radiologically and electro - otologically. These patients manifested a fluctuat-ing hearing impairment particularly in low fre-quency tones. Canal paresis on the affected side were observed in all cases.

The vertebral angiography demonstrated that the vertebral artery on the affected sides showed a large -diameter and a sharp-angulation at the fourth segment.

These findings seemed to suggest the inner ear lesion due to vertebro-basilar artery insuffiency.
