アレルギー性鼻炎に対する炭酸ガスレーザーによる鼻腔粘膜焼灼術

CO2 Laser Vaporization of Nasal Mucosa in Patients with Allergic Rhinitis

　はじめに

　アレルギー性鼻炎に対しては，薬物，減感作療法，手術などいろいろな治療法が行われている。なかでも非吸収性ステロイドの点鼻薬，抗アレルギー剤を中心とした薬物治療は，アレルギー性鼻炎の基本的な治療となっている。さらに，薬物治療にも抵抗する患者に対しては，以前より種々の手術療法や星状神経ブロックなどの治療が行われている。手術治療のうち炭酸ガスレーザーによる下鼻甲介手術は須藤ら^1），福武ら^2），友田ら^3）が有効な治療法であると報告している。著者らは前者らとは異なり，1回の焼灼術にてどの程度の効果が得られるかを主目的として，保存的治療に抵抗する通年性アレルギー性鼻炎患者に対して，外来での炭酸ガスレーザーによる鼻腔粘膜焼灼術を施行した。今回は56症例において自覚症状の術前術後の変化を問診し，本治療法の有効性，手術効果について検討したので報告する。

CO₂ laser vaporization of nasal mucosa, using a hand piece, was performed once in 10 mi-nutes for 56 patients including 40 children under 12 years old with allergic rhinitis. All the pa-tients presented with symptoms in more than mild degree, positive to house duct allergens and intractable to conservative treatment. In the ob-servation period of 4 to 5 months, improvement of symptoms were obtained in 82.1% and effect of the treatment was sustained for 15.9 weeks in average. Complications were not remarkable.
