当院における通年性アレルギー性鼻炎に対する炭酸ガスレーザーによる短期治療成績

Treatment results with the carbon dioxide laser for the perennial allergic rhinitis in this clinic

Ⅰ　はじめに

　アレルギー性鼻炎に対しては薬物療法，減感作療法，外科療法などが挙げられる。保存的治療に対して難治な場合や，保存的治療の継続が困難な症例に対しては外科療法が選択されている。レーザー治療は外科治療のうち外来にて施行可能な方法であり，炭酸ガスレーザー，半導体レーザー，YAGレーザーなどが臨床的に使用されている。このうち，レーザー機器の操作性，価格，術後副反応の少なさから，外来手術としては炭酸ガスレーザーが最も普及しており，有効性，安全性についてもほぼ確立されている。炭酸ガスレーザーではレーザー光の組織深達度が0.5mm以下であるため，大部分のエネルギーが表面から0.1～0.2mmの粘膜表面にて吸収される。このため深部組織を傷つける危険性が少ないという安全性がある。この一方，パワーが弱く1回の焼灼では不十分であることもある。有効率としては鼻閉90％，鼻汁85％，くしゃみ70％と報告^1）されている。今回当院において薬剤無効の通年性アレルギー性鼻炎症例に対して炭酸ガスレーザーによる下甲介粘膜焼灼術を行った症例について同時に一側のみ焼灼術を行った場合と，両側を行った場合との比較を中心に成績の検討を行った。

　The inferior turbinate mocosa was vaporized with the carbon dioxide laser for perennial allergic rhinitis cases in which medicine was invalid and the results were evaluated as curative. At the same time,we compared with the cases that performed a laser surgery in both sides to the cases in one side. The effective rate 2 months postoperatively was 85％ in nasal obstruction,72.7％ in nasal rhinorrea,78.9％ in sneeze. In comparison with the other report,my treatment results were approximately at the same level. I compared VAS scores before and after surgery according to symptoms. In case that the main complaint was nasal obstruction,we thought that bilateral laser surgery should be carried out. In addition,if the main complaint was rhinorrhea or sneezing,it was considered to be effective in the laser surgery of one side only.
