全身性エリテマトーデス（SLE）の神経耳科学的検討

はじめに

　膠原病は中枢から末梢にわたる多彩な神経症状を呈することが知られており，時にめまい・平衡障害を起こす。われわれ耳鼻咽喉科医が最終的に膠原病を診断する機会はないが，他科で診断された患者がめまい・平衡障害を起こして耳鼻咽喉科にコンサルトされることがある。しかし，その詳細についての報告は稀である。今回，北里大学病院耳鼻咽喉科めまい外来および北里大学東病院神経耳科を受診した，膠原病の代表的疾患である全身性エリテマトーデス（systemic lupus erythematosus：SLE）患者の神経耳科学的所見について検討したので報告する。

　Collagen disease is known to present various neurological symptoms from the peripheral to the central nervous system（CNS）, and it may sometimes cause vertigo and disequilibrium. Here, I present a retrospective study of patients with systemic lupus erythematosus（SLE）, a representative of collagen disease. We selected 25 out of 10,762 SLE patients who visited the specialized vertigo outpatient clinic. As a result, 9 patients had CNS disorders, 6 patients had Meniere's disease, and 4 patients had benign paroxysmal positional vertigo. It has been suggested that there are various pathogenesis and lesion sites in patients with SLE.
