三叉神経痛ならびに顔面痙攣に対するS状静脈洞後方からのアプローチによる神経血管減圧術

Neurovascular Decompression by Retrosigmoidal Approach for Trigeminal Neuralgia and Hemifacial Spasm

I．はじめに

　三叉神経痛と顔面痙攣は中・高年者に比較的多くみられる疾患である。1977年Jannettaら^1）が顔面痙攣に対し神経血管減圧手術が有効であることを詳細に報告して以来，多くの研究者がこの術式を追試してきた^2〜4）。

　従来からの方法は主として迷路の後方から神経血管に到達する方法であったが，著者らは1985年以来，S状静脈洞の後方から到達する力法で手術を行い，良好な成績が得られたので報告する。

Trigeminal neuralgia and hemifacial spasm are seen in middle or older aged persons. In sixty cases of trigeminal neuralgia and forty-five cases of hemifacial spasm the trigeminal or facial nerve was decompressed by retrosigmoidal approach. Sur-gical procedure is described. Compression of the nerves was noticed in all these cases at operation. One hundred and four patients (99.05%) were free of symptoms postoperatively, confirming the pathology reported in other clinical reports. Path-ologies producing the compression and its sites arc discussed.
