治療に難渋した喉頭悪性線維性組織球腫の1例

Malignant fibrous histiocytoma of the larynx：a case report

Ⅰ．はじめに

　悪性線維性組織球腫（malignant fibrous histiocytoma：MFH）は中胚葉由来の軟部組織に発生する悪性腫瘍である。頭頸部領域での発生は3～10％であるが，その中では副鼻腔原発が最も多く約30％を占め，喉頭原発は10～15％と，比較的まれである^1）。今回われわれは後期高齢者に発症し，経過中さまざまな合併症を併発したため，治療に難渋した喉頭MFHの1例を経験したので，若干の文献的考察を加えて報告する。

　Malignant fibrous hystiocytoma（MFH）is the most common soft tissue sarcoma in adults. But MFH of the larynx is rare. We reported a case of a larynx MFH in a 87-year-old man. We performed laryngomicrosurgery to remove the tumor.

　Before the laryngeal treatment,he developed rupture of the bladder and intestinal obstruction. After the abdominal surgery,the radiation therapy was performed. Especially elderly patients who had some past medical history,we should take care of their general condition and select the appropriate treatment.
