反回神経麻痺と喉頭癌

RECURRENT LARYNGEAL NERVE PARALYSIS AND LARYNGEAL CANCER

　I．はじめに

　反回神経麻痺のために運動が固定した声帯から発癌がみられるか否かについては解答は得られていない。著者は運動しない声帯は安静が保たれているので発癌しにくいのではないかと考えてきたし，現在もそう想像している。ところが本論文で述べる2症例は明らかに反回神経麻痺と声帯癌が合併した症例であつて，症例1は肺切除術後の反回神経麻痺声帯に術後13年目に声帯癌が発生し，症例2はまず声帯癌が発生し，約1〜2カ月後に合併重複していた食道癌のために反回神経麻痺が同側に発症した例である。

　人体における反回神経麻痺の病理は未解決であり，これから症例の積み重ねが必要である。したがつて麻痺声帯の発癌母地の価値考察は推測の域をでないが，あえて検討を行なつたものである。

　Two cases of laryngeal cancers with recurrent laryngeal nerve paralysis are reported. In case 1 the cancer developed on the paralytic vocal cord. In case 2 the vocal cord paralysis occurred in consequence of and a concommitant esophageal cancer. In both cases the cancers were in stage I development but they were diagnosed, elsewhere, as being in the stage III.

　Pathological aspect of the recurrent nerve paralysis are discussed.

　The author is of opinion that possibility of cancer formation on paralytic vocal cord is considered as a very rare occurrence.
