気管内挿管チューブの不適合が原因の声門下狭窄症例

Subglottic stenosis due to incompatibility of intubation tube

I．はじめに

　声門下狭窄の原因として，気管内挿管，外傷，炎症性，特発性などが挙げられる。外科的治療が行われることが多いが，個々の症例の臨床症状や病態に応じて治療方針を決めなければならない。今回われわれは，抜管直後から嗄声が生じた症例で声門下狭窄と診断したが保存的治療で治癒したので，若干の文献的考察を加えて報告する。

A 78-year-old woman was complaining difficulty breathing. She was admitted to emergency department because of unconsciousness and she was diagnosed of heat stroke. She received a tracheal intubation tube of 8.5mm immediately,which was changed to a 6.5mm tube after hospitalization. Hoarseness developed just after and we diagnosed subglottic stenosis due to intratracheal intubation. she was treated with administration of oxygen,intravenous steroid infusion and inhalation. After for a few days,an operation was planned because she showed no improvement. However,her subglottic stenosis disappeared preoperatively.
