結膜下膿瘍を眼症状としたWegener肉芽腫症の1例

A case of Wegener's granulomatosis with subconjunctival abscess

（展示263）　結膜下膿瘍を眼症状としたWegener肉芽腫症の1例を経験した。症例は73歳の男性で，右眼視力低下と右下鼻側結膜の腫瘤性病変を主訴として受診した。腫瘤部の病理組織診断では好中球の混在した壊死組織であった。血中抗好中球細胞質抗体（cANCA）陽性であること，肺部結節の生検で肉芽腫性病変をみたことから，Wegener肉芽腫症と診断された。ステロイドパルス療法および免疫抑制剤投与により全身状態は改善した。同時に右眼結膜下膿瘍は縮小するとともに，その下に壊死性強膜炎の存在が明らかとなった。

A 73-year-old male presented with gradual loss of vision and subconjunctival abscess in the right eye. Systemic examinations showed lung tumor by X ray, granulomatous lesion by lung biopsy, elevated CRP and leucocyte count and positive cANCA. These findings led to the diagnosis of Wegener's granulomatosis. Systemic corticosteroid resulted in improvement of ocular lesion but not of the lung. Signs of necrotic scleritis became manifest after resolution of the subconjunctival abscess.
