増殖性硝子体網膜症に対する放射線療法の試み

A preliminary study of radiation therapy for proliferative vitreoretinopathy

　4例4眼の増殖性硝子体網膜症について，硝子体手術後に20Gyの放射線治療を行った。術直後に，角膜上皮欠損の遷延化と，やや強い前房内フィブリン反応が2眼に認められたが，10日から2週間で消退した。平均6か月間の経過観察中に放射線照射によると考えられる合併症はなかった。4眼中2眼で，網膜が復位し，最終視力は良好であった。術中に網膜の部分復位しか得られなかった2眼は，術後も網膜は部分復位の状態であり，有用な視力は得られなかった。放射線療法は，増殖性硝子体網膜症の発症，進展を抑制する補助療法として使用できる可能性があると評価された。

A preliminary study of beta-irradiation for proliferative vitreoretinopathy (PVR) was done. A total dose of 20 Gy (2 Gy/day) was applied to 4 eyes of PVR after vitrectomy. In 2 eyes, the retina was reattached, while only partial reattachment was obtained in 2 eyes. Possible immediate side effects of the radiation include persistent epithelial defect of the cornea and stronger fibrin reaction in the anterior chamber. No late complication was observed in the average follow-up period of 6 months. This preliminary study showed that beta-irradiation of 20 Gy is not hazardous to eyes of PVR for at least 6 months. Radiation therapy may be a promising adjunctive treatment for PVR.
