一過性霧視発作をきたした巨大乳頭の1例

Megalopapilla associated with recurrent amaurosis fugax

　24歳の女性で全身運動時に片眼の一過性霧視発作を頻発する症例に巨大乳頭を認めた。

　初診時の視力は良好であった。視野は患眼のマリオット盲点の拡大がみられた。

　患眼の眼底には乳頭の著しい拡大と乳頭周囲の白色輪状組織および網膜静脈の著しい拡張・蛇行がみられた。他眼には異常をみなかった。螢光眼底造影により，乳頭周囲の白色輪状組織は造影早期は低螢光，晩期には過螢光を示した。また，網膜静脈の還流が遅れ，うっ滞を示す所見がみられた。

　本症例は，乳頭からその周囲に，組織の過剰増殖により，網膜動静脈に相対的に絞扼が起こり，網膜動静脈の循環不全状態になっていて，全身運動による上半身への血流低下のために血行不全が発生し，—過性霧視発作を起こしたものと思われた。

A 24-year-old female sought medical advice on account of frequent temporary attacks of blurring of vision in the left eye during exercise since 10 years of age. The corrected visual acuity was 20/20. Campimetry showed enlarged blind spot. Fundus-copy showed the optic disc larger by 50%than in the right eye. A white fibrous ring was located around the disc. The retinal veins were tortuous and dilat-ed. Fluorescein angiography showed hyperfluores-cence of the white ring and marked stasis in the retinal veins.

It appeared that the amaurosis fugax was caused by a state of incomplete venous occlussion due to constriction of the disc by the fibrotic ring tissue. Physical exercise would trigger temporary occlu-sion of retinal veins through reduced intraocular blood flow.
