長期経過観察の可能であった定型的網膜色素変性患者の視野とERG

Visual field and electroretinogram in typical retinitis pigmentosa

　網膜色素変性患者（散発例と常染色体劣性）の視野欠損の進行が網膜機能をどのように反映しているか，ゴールドマン視野計によるV−4およびI−4視野の面積とERGとを比較することにより検討した。I−4視野と錐体機能を表すphotopic ERGは遺伝形式の有無にかかわらず年齢とともに低下し，I−4視野が広いほどphotopic ERGの電位は大きくなっていた。一方，V−4視野では散発例のみに年齢に従った視野欠損の進行が認められ，散発例の1例以外はすべて検出することが不可能であった。

We reviewed the clinical course in 10 patients with typical retinitis pigmentosa over a period of 5 years. The ages ranged from 9 to 33 years. The disease was sporadic in 6 cases and autosomal recessive in 4. Particular attention was paid to the amplitude of summated ERG and the visual field area as tested by 4-I and 4-V targets of Goldmann perimeter. The amplitude of photopic ERG, which reflects cone functions, decreased in both sporadic and hereditary cases. This decrease was age-dependent. The amplitude of photopic ERG was higher in eyes with wider visual field for 4-I target. The visual field for 4-V target was age-dependently progressive in sporadic cases. Scotopic ERG, which reflects rod functions, was nonrecorda-ble throughout except in a sporadic case.
