From a review of 1,800 cataract surgeries, we could confirm the usefulness of topical injection of 1ml of 2% lidocaine into the Tenon's capsule. This procedure was safer than conventional retrobulbar anesthesia because of absence of pain and lesser incidence of complications including retrobulbar hemorrhage. It could also be learned with ease and be administered even by a beginner surgeon. It was better tolerated by the patient and did not induce rise of systemic blood pressure or arrythmia. While this procedure does not repress ocular motility, this feature did not noticeably affect the surgical mani-pulation for cataract and intraocular lens implanta-tion.
