膠様滴状角膜変性症再発例に対し表層角膜移植および角膜上皮移植術を施行した4例

Lamellar keratoplasty combined with keratoepith-elioplasty in four cases for recurrent gelatinous drop-like corneal dystrophy

　膠様滴状角膜変性症再発例の4症例に表層角膜移植術および角膜上皮移植術を施行した。経過観察期間は5か月から1年6か月であった。1例に軽度の角膜上皮欠損を，もう1例に移植片全体におよぶ遷延性角膜上皮欠損を認めた。後者は上皮下に混濁を残したが，他の3例は透明であり全例に再発を認めていない。異物感，流涙，羞明などの自覚症状もかなり改善された。使用した角膜は70〜88歳であるが，本術式は角膜内皮側を使用しないために，比較的高齢者の角膜でも十分に使用可能である。他の角膜疾患より再発が多い本疾患に対し，本術式は有効な方法と思われた。

We performed lamellar keratoplasty combined with keratoepithelioplasty in 4 cases with recurrent post-lamellar keratoplasty of gelatinous drop-like corneal dystrophy. The postoperative follow up period ranged from 5 to 18 months. The corneal surface remained clear in 3 cases. One case showed prolonged epithelial defect during the observation period. There was no recurrence of amyloid throughout the cases. Donor cornea was used with-out endothelial layer, so it is possible to use aged cornea for lamellar keratoplasty and keratoepith-elioplasty. It seemed to be effective for recurrent post lamellar keratoplasty of gelatinous drop-like corneal dystrophy.
