網膜血管増殖性腫瘍による続発性黄斑上膜を認めた2例

Two cases of secondary epiretinal membrane associated with retinal vasoproliferative tumors

要約　目的：網膜血管増殖性腫瘍（RVPT）は網膜血管およびグリア細胞の増殖を示す比較的稀な疾患である。今回，RVPTによる続発性黄斑上膜を認めた2例を経験したので報告する。

症例1：62歳，男性。左眼に黄斑上膜および網膜周辺部耳下側に橙黄色腫瘤を認めた。腫瘤に対し網膜光凝固術を施行し，術1か月後に黄斑上膜が増強したため，術2か月後に硝子体手術を施行した。摘出した黄斑上膜は，免疫染色で血小板内皮細胞接着分子-1（CD31/PECAM-1），GFAP染色，α-SMA陽性，PDGF-β陰性であった。

症例2：55歳，女性。右眼は黄斑上膜および網膜周辺部下方に血管腫様の腫瘤を認めた。硝子体手術にて黄斑上膜の剝離除去を行ったRVPTに対しては腫瘍を凝固壊死させる目的で冷凍凝固術を行った。摘出した黄斑上膜は，免疫染色でCD31，GFAP陽性であった。

結論：RVPTに続発する黄斑上膜に対し硝子体手術で黄斑上膜を除去し，組織学的検索を行った。除去した黄斑上膜は未熟な血管成分がみられ，糖尿病網膜症の増殖膜と類似する所見が得られた。このことから本症における黄斑上膜は急速に肥厚し拡大する可能性があるため，早期に黄斑上膜の剝離除去を行うことが望ましいと考えられた。また肥厚し，網膜に強固に癒着した黄斑上膜を診た場合，網膜周辺部に血管腫瘍が存在する可能性を考え，注意深く眼底検査を行うべきである。

Abstract　Purpose：Retinal vasoproliferative tumors（RVPT）is a relatively rare disease involving proliferation of retinal bold vessels and glia cells. We report two cases of secondary epiretinal membrane（ERM）associated with RVPT.

Case 1：A 62-year-old man presented with an RVPT and a macular ERM. The ERM became thicker with the development of vitreoretinal traction after laser photocoagulation of the RVPT. We then performed vitrectomy and peeled the ERM.

Case 2：A 55-year-old woman presented with an angioma like tumor and a macular ERM. We performed vitrectomy to peel the ERM and trans-scleral cryotherapy for the RVPT.

Conclusions：We treated and histopathologically examined two cases of ERM associated with RVPT. In both cases, we observed positive staining of the ERM for CD31 and GFAP；in case 1, negative staining for PDGF was found but on hematoxylin eosin staining of the ERM, the presence of vascular lumina, endothelial cells, and pericytes was not confirmed. The findings are indicative of immature vascular components and similar to those of a proliferative membrane found in diabetic retinopathy. The ERM can proliferate rapidly if secondary to an RVPT. We therefore considered that early peeling of the ERM could improve the prognosis.
