Abstract Introduction:The pathogenesis of acute zonal occult outer retinopathy(AZOOR)is still unclear, but it has been suggested that autoimmunity may be involved. Anti-aquaporin(AQP)4 antibodies are widely known to be involved in the development of neuromyelitis optica spectrum disorders(NMOSD). We report a case of anti-AQP4 antibody-positive AZOOR treated with steroid pulse therapy.

Case:A 30-year-old man suddenly became aware of visual field loss in the right eye. His symptoms worsened over time, and optic neuritis was suspected at another hospital;he was then referred to our hospital 11 days after the onset.

Findings:Corrected visual acuity was 0.5 in the right eye, and 1.2 in the left eye, and the right visual field was extensively deficient temporally. Optical coherence tomography showed disruption of the ellipsoid zone in his right eye, and multifocal electroretinogram revealed a potential drop consistent with visual field defects. He was diagnosed with AZOOR. After intravenous administration of 500 mg/day of methylprednisolone for 3 days, oral administration of prednisolone was continued and the visual field expanded. After treatment initation, a blood test revealed positivity for AQP4 antibodies. However, but there were no findings suggestive of NMOSD on a repeat MRI.

Conclusion:We experienced a case of AZOOR that was positive for anti-AQP4 antibodies. Improvement in visual function was obtained after systemic administration of steroids. There were no findings suggestive of concomitant NMOSD.
