Abstract Purpose:To evaluate the effect of iStent® insertion on lowering intraocular pressure(IOP)and reducing glaucoma medications.

Subjects and methods:Thirty eyes of 23 patients who underwent iStent® insertion combined with cataract surgery at the Department of Ophthalmology, Toranomon Hospital were included. We evaluated reductions of IOP and glaucoma medications in two groups:the high group(17 eyes)with preoperative IOP of 20 mmHg or higher and the low group(13 eyes)with IOP of less than 20 mmHg, and also performed survival analysis.

Results:The mean follow-up period was 17.1±9.7 months. The mean pre- and postoperative IOPs were 21.0±4.9 mmHg and 16.1±5.2 mmHg, respectively, which were significantly different(p=0.000036). The median range of IOP reduction was 7.0 mmHg in the high group and 2.3 mmHg in the low group, respectively. The pre- and postoperative glaucoma drug scores were significantly decreased by 0.76 in the high group(p=0.014)but not in the low group(p=0.090). The percentage of patients who survived at the final follow-up was 53.8% and 19.4% in the high and low groups, respectively, which was significantly different(p=0.0054). Three eyes in the high group required additional glaucoma surgeries.

Conclusions:Insertion of iStent® may be useful in lowering IOP and decreasing glaucoma medications. It is suggested that iStent® is more effective in less advanced glaucoma with higher IOP.
