乾癬に併発した網膜動脈閉塞症の1例

A case of retinal artery occlusion associated with psoriasis

要約　緒言：乾癬は皮膚における慢性の炎症性角化症である。自己免疫が発症に関与し，近年は生物学的製剤の有効性が知られている。乾癬には全身の血管障害を併発することがあるが，網膜血管障害を生じることはあまり知られていない。今回，乾癬に伴う網膜動脈閉塞症例を経験したため報告する。

症例：55歳，女性。近医での定期検査時に左上鼻側の網膜動脈の白線化を指摘され，当科を紹介され初診となった。初診時の左矯正視力は1.2であった。既往歴として乾癬があり，皮膚科にてアダリムマブの定期投与中であった。本剤による副作用を疑い，イキセキズマブに変更し経過観察となった。初診から10か月後，矯正視力1.2であったが左下鼻側に網膜動脈分枝閉塞に伴う霧視を自覚し，再診となった。原疾患以外の動脈閉塞の原因精査を行ったが明らかな異常はなく，1週間後の再診時に網膜動脈閉塞範囲の拡大を認めた。前房穿刺，ウロキナーゼと硝酸薬の投与を行い，加療後の左矯正視力は0.4であった。

考按：乾癬は網膜血管障害を合併することがあり，注意が必要である。

Abstract　Purpose：Psoriasis is a chronic autoimmune disease characterized by the formation of inflammatory plaques on the skin. Recently, biological therapies using monoclonal antibodies have been developed. Although the association of cardiovascular disease or arteriosclerosis with psoriasis is known, its association with retinal vascular disease is unclear. Herein, we report a case of retinal artery occlusion associated with psoriasis.

Case：A 55-year-old woman was referred to our clinic due to a finding of white retinal artery in her left eye. At initial visit, decimal visual acuity of the left eye was 1.2. She previously had psoriatic arthritis and had received adalimumab treatment at the department of dermatology. Because an adverse effect due to adalimumab was suspected, adalimumab was switched to ixekizumab. Ten months after the initial visit, the patient developed retinal artery occlusion with blurred vision in her left eye, though the decimal visual acuity remained 1.2. Despite detailed examination, the occlusion area spread 1 week later. The decimal visual acuity of the left eye remained 0.4 after treatment.

Conclusion：Cases of retinal vascular disease associated with psoriasis warrants special attention.
