Abstract Purpose: To report the relation between fluctuation in the time of instillation and adherence to glaucoma medication.

Cases and Method: This study was made on 324 glaucoma patients. The series comprised 138 males and 186 females. The age averaged 70.6±11.6 years. They were prescribed with either latanoprost 0.04% or fixed combination of latanoprost 0.04% and timolol 0.5% for 6 months or longer. Data regarding the time of instillation and adherence was obtained by interview.

Results: The time of instillation fluctuated by an average of 60.0±74.3 minutes. It fluctuated by 52.3±62.1 minutes in good adherents and by 78.3±94.3 minutes in poor adherents. The difference was significant(p<0.0001). Patients who reported to have no fixed time for instillation showed fluctuation of 227.5±161.0 minutes. This value was significantly higher than the rest in the present series(p<0.0001).

Conclusion: Adherence to glaucoma regimen deteriorated significantly parallel with the fluctuation in the time of instillation.
