ステロイドで視力改善した側頭動脈炎の確定診断例

Systemic corticosteroid induced visual improvement in a case with definite temporal arteritis

　生検により確定診断した側頭動脈炎で，右眼虚血性視神経症を伴った症例に，ステロイド治療を行った。後頭部痛で発症し，初診時視力は指数弁で，視神経乳頭には著明な蒼白浮腫を認めた。迅速な診断のもとにプレドニソロン80mgから開始したステロイド治療により，視力は0.5まで改善したが，視野は中心から耳側のみを残して著明に狭窄したままで，改善をみなかった。右浅側頭動脈は生検により巨細胞性動脈炎の所見を示した。高齢者の頭痛と視神経乳頭の蒼白浮腫を認めた際には，側頭動脈炎を忘れてはならず，早期に確定診断の努力とステロイド治療を行うべきである。

A 71-year-old male visited us a few hours after headache with acute visual loss in his right eye. Funduscopy showed paleness and swelling of the disc in the affected eye. Visual acuity was finger counting. The findings were compatible with acute ischemic optic neuropathy. Biopsy of the superficialtemporal artery showed arteritis with giant cell infiltration and led to the diagnosis of temporal arteritis. He was put under treatment with systemic prednisolone, 80 mg per day, from the first day of ocular involvement. The visual acuity improved in 3 weeks. Early diagnosis and initiation of treatment seemed to have contributed to the improvement.
