7歳女児の右眼に5日前から充血があり,フィブリン析出を伴う強い虹彩毛様炎と樹氷状血管炎が初診時に見られた。ステロイド剤の全身投与で,炎症は2ヵ月後に消褪し,下鼻側周辺に隆起性病変を残した。発病後2年3ヵ月の間,再燃はない。
血清学的検査は弱陽性であったが,典型的な臨床像と合せ,眼犬蛔虫症と診断した。
A 7-year-old girl developed acute unilateral fi-brinous iridocyclitis and retinochoroiditis in her right eye. On day 5 of the disease, we observed diffuse retinal vasculitis simulating frosted branches in the affected eye. During the ensuing period, we noted gradual elevation of yellowish-white exudates in the inferonasal periphery. Vitre-ous bands formed extending to the optic disc.
Serum antibody to toxocara canis was weakly positive by ELISA test. The intraocular inflamma-tion promptly responded to topical and systemic corticosteroid, resulting in complete disappearance of frosted retinal vasculitis. We observed no recur-rence or tractional retinal detachment during the follow-up period of 17 months. Frosted retinal vasculitis seemed to be an early funduscopic mani-festation of ocular toxocariasis.
