嚢胞状黄斑部浮腫に対する高気圧酸素治療法

Hyperbaric oxygen treatment for cystoid macular edema secondary to branch retinal vein occlusion

　網膜分枝静脈閉塞症に合併した嚢胞状黄斑部浮腫5症例に対して高気圧酸素治療を施行した．2ないし3週間の高気圧酸素治療により，5例全例に視力および静的視野閾値の改善が認められた．改善した視機能は治療を中止しても，良好なまま維持される傾向を示した．その奏効機序としては，網膜血管外漏出の減少，好気性代謝の活性化による網膜細胞の賦活，化学伝達物質の阻害などが推測されたが，詳細は不明であった．今後，より長期的な経過観察を要するが，高気圧酸素治療法は嚢胞状黄斑部浮腫に対する視機能改善の方法として有効な治療手段であると考えられた．

Five cases with cystoid macular edema secon-dary to branch retinal vein occlusion were treated with hyperbaric oxygen. The patients underwent total body hyperbaric oxygen treatment in a cham-ber set at 2 atmospheres for one hour twice daily, for two or three weeks. The visual acuity improved in all the cases following treatment and tended toremain stable. It was speculated that hyperbaric oxygen might reduce the vascular leakage, activate aerobic metabolism, and inhibit inflammatory chemical mediators. Although the exact mechanism is still unclear, hyperbaric oxygen treatment prom-ises to be an effective modality for established cases of macular edema after branch retinal vein occlusion.
