白内障水晶体の徹照像撮影

Retro-illumination photography of cataractous lens

緒　言

　白内障水晶体の混濁状態を混濁形成の追跡および混濁程度の定量的測定の方面から正確に把握することは，白内障の臨床上重要なことである。この目的で細隙灯生体顕微鏡による検査や視力による評価が現在行われている。しかしながら，細隙灯生体顕微鏡による観察所見を的確に表現・記録することは容易ではなく，写真撮影においても局部的な記録のみが可能である。また視力を尺度とする判定法は混濁程度以外の要因によつても変動しうる可能性があり，あくまで自覚的な評価であるため客観性に乏しいと考えられる。

　自内障水晶体の徹照像は，混濁部を網膜からの反帰光線の影として表わし，平面的にではあるが水晶体全面を一度に観察・撮影できる利点を持つている^1〜4）。そして林ら^2）およびHockwinら^3）は，白内障の混濁経過の評価に徹照像が有効に利用できると述べている。しかしながら既存の徹照写真では，光源像などの角膜からの反射光が観察の邪魔となり定量的解析が困難であつた。

A new device For photographing the catarac-tous lens opacities with retro-illumination has been investigated.

The Photo-Slit Lamp (Carl Zeiss) was modi-fied with newly deviced adaptor to eliminate the reflection from the corneal surface. The sub-ject's eye was illuminated with horizontally po-larized beam. The lens opacities were photogra-phed on the same optical axis to the illumination beam through a polaroid which axis of polarization was vertical. Additionally an orange filter was used to increase the image contrast.

The results are satisfactory with high resolu-tion and good contrast.
