眼窩内転移を来たした皮膚悪性黒色腫の1例

Metastasis of Melignant Melanoma of the Sink to the Orbit. Report of a Case

緒　言

　一般に，悪性腫瘍の眼窩内転移は比較的稀なものとされている上に，皮膚悪性黒色腫は皮膚悪性腫瘍の数％を占めるにすぎないものである^1）ことからみて，その眼窩内転移は極めて稀なものと推測される。著者の調査した範囲では悪性黒色腫の眼窩内転移例として，1894年のPolignaniの1例^2）を嚆矢とし，16例を数えるが，そのうちで皮膚を原発とすることを記しているものは9例にすぎない。

　著者は皮膚を原発とし，全身性転移を呈して死亡するに至る経過中に，左眼眼球突出を来たし，死亡後，眼窩内における悪性黒色腫の転移巣を確認しえた症例を経験したので報告する。

A 41-year-old female developed metastatic malig-nant melanoma of the left orbit originating from skin malignant melanoma of the hip region. The orbital lesion was associated with exophthal-mos, ocular hypotony and increase of soft tissue density in orbital roentgenogram. Transient hypo-sphagma developed before the exophthalmos became manifest. The ocular movement was not disturbed throughout the clinical course. Autopsy proved the metastatic tumor to be located within the muscle cone of the orbit.

Orbital metastasis of malignant melanoma of the skin is a rather unusual event. Only 9 cases have been reported in literature.
