2．ト手術後の結膜変化及びその分類

　此の様な手術により結膜変化を模型的に整理すると第1図の如くなる。以下簡単に総括説明を加える。

（1）前垂形成型（前編症例1）最近は減少したがパクレン，トラコーマ鑷子等に依り強度に結膜を損傷し，主に膠原層迄侵された揚合，之が基礎となり発生すると考えられる。或いは強度の偽膜（或いは濃厚なベラーク）を作つた場合これの組織化が行われ，その下に濾胞，細胞浸潤が存在し，未治癒の状態となる。発生予防には勿論此の様な強度の手術を行わないことと，偽膜発生の場合その組織化の前に剥離を必要とする。

The key factors causing refractory trachoma were assumed to lie in 1. Acquisition of resistance against treatment by the trachomatous tissue. 2. Fastness and ransmutation of the virus. 3. Repeated recurrence (Report Ⅱ). 4. Excessive or inappropriate surgical intervention. Some relevant points were discussed on this assumption.

Some authors have already reported on the observed fastness of trachoma virus, but the present authors believes the problem is yet to be studied and still unripe for definite an-swer.
