（9）クラーレの眼輪筋に及ぼす影響のEMGによる観察

Observation from EMG of Influences upon M. orbicularis oculi by Clare.

　現在，白内障の弁状摘出に当つて，嚢内摘出を常套手段とするべきであることは当然である。この嚢内摘出を行うにあたつて，第1の危険が硝子体の脱出である。昨年D．B．Kirby等が集つての白内障手術のシンポジウム^1）に於いて，この第1の危険などを考慮して，嚢内摘出術にはクラーレによるアキネジアが不可欠のものであることを決定している。

　アキネジアの目的にクラーレを使用すると手術操作を容易にし，手術効果をたかめることは既に昨年私が報告^2）したところである。以後，私は約100眼の弁状摘出にクラーレを使用しているのであるが，その間投与量に関して多少の疑義を生じた。即，過投与は全身的な危険に迄は到らなくても，嚢内摘出の際，嚢の把握及び摘出の過程に幾分困難を感じる場合があるからである。従つて筋の張力の微細な変化を時間的に追求し，アキネジアの発生状態及び回復状態を観察する目的で，眼輪筋の張力の変化をEMGにより観察した。

It is very well known that the intracapsular extraction occupies the primary position in the flap extraction of cataract. In order to facilitate this extraction easily akinesia with curare is an important meaus and must be conducted with adequate caution. As some doubts arose about the dosages of curare to be used for akinesia which I reported last year, I studied a new degrees of M. orbicularis akinesia in terms of E.M.G. The following is the result of studies thus conducted on the appropriate doses and methods of dosage.
