特異な続発性網膜剥離—漿液性中心性脈絡膜症（増田）および周辺性ブドウ膜炎との関連

A Peculiar Type of Secondary Detachment of the Retina. With Special Reference to Central Serous Choroidopathy and Peripheral Uveitis

緒言

　続発性網膜剥離の原因としては種々の疾患が列挙されているが，脈絡膜からの著しい滲出による高度な網膜剥離を伴い，日常比較的よく遭遇する疾患としては，わが国では原田病がよく知られている。原田病は特徴的な臨床所見を呈するので，診断を誤ることはまずない。この疾患をのぞいて最近話題となつているものに周辺性ブドウ膜炎がある。わが国でも周辺性ブドウ膜炎に関する報告^5）〜9），解説^5）が少なくなく，眼科医の関心をあつめている。

　著者らは，これらの疾患とは別の疾患で，不明な原因によつておこる高度な続発性網膜剥離が存在することを推定している。本稿ではその発病機序，原因を推定するのに好都合と思われる2症例を選択供覧し，その発病機序に関し，漿液性中心性脈絡膜症（増田）（増田型漿液性中心性脈絡網膜炎）と関連づけて考察してみたい。

This paper reports two cases of peculiar type of secondary detachment of the retina.

In the first case a 47-year-old man developed metamorphopsia of the left eye 3 months ago and was treated as retinal hemorrhage for 2 months wrthout mprovement. This time he was told to have detached retina in the left eye and referred to us on February 16, 1971. There was no pathology in the anterior segment of the eye. The aqueous was clear and the iris was normal. There were a few floaters in the vitreous but no inflammatory exudates were observed in the vitreous.
