粘液嚢腫に対する脂肪充填術

Implantation of Adipose Tissue Following Surgery for Myxoma of the Frontal Sinus

I．緒言

　副鼻腔粘液嚢腫（粘液瘤）は発生頻度から見ると，前頭洞に最も多く，ついで篩骨洞である。そのため症状はほとんど前額部や上眼瞼の腫脹，眼球突出，瘻孔形成，前頭骨のOsteomyelitisなどに限られている。進行して眼窩上壁を吸収すると，眼球突出，眼筋まひ，眼窩蜂窠織炎，球後視神経炎などの眼症状を起こす。このように眼周囲の腫脹から始まつて種々の眼障害を起こしてくるので，眼科的に処置せねばならない場合が生ずる。このさいわれわれが直面する問題は，いかにして副鼻腔内に粘液，膿の再貯溜，嚢腫の再発を予防するかということである。そこでわれわれは，2例の粘液嚢種に対してこれを摘出し，その後の副鼻腔に自家脂肪を充填する手術を行なつて好結果を得たので報告する。

　Two cases with myxomatous cyst of the frontal sinus had been operated on in whom extensive destruction of the bone was present in the medial and the upper orbital margin. Satisfactory cosmetic effect and prevention of relapse could be achieved by removal of the cyst and the mucous membrane of the frontal sinus and by filling up the resultant cavity with adipose tissue obtained from the abdomen of the patient. The latter procedure is recommended because of its efficacy and easiness.
