Cardiac Glucoside in the Treatment of Chronic Glaucoma

I．緒言

　急性ウツ血性緑内障については，手術的療法が推賞されているが，その他の慢性の緑内障については適確な治療法がなく，最初から手術的に眼圧の調整を図るよりも，まずピロカルピンを初めとする縮瞳剤によつて眼圧の調整を試み，これが不能となつたときにはじめて手術を行うべきであるという考え方が有力である。

　手術療法は房水排出系の障害を緩和する方法が主力を占め，多くの手術方法がある。これら濾過手術は理論的には甚だ理にかなつてはいるがその効果についてはまだ十分とはいい難い。それ故に，上の縮瞳剤の他に，更に房水排出系の障害を改善する薬剤，或いは次善の策として房水の産生を抑制する薬剤の出現が期待される。

Cardiac glucoside is reported to inhibit the activity of Na-K-ATPase of ciliary body to reduce the aqueous formation.

In this paper six cases of chronic glauco-matous patients treated with digitoxin for two to seven months were presented and digitoxin is concluded to be an excellent drug in the long term treatment of chronic glaucoma.

Through oral administration of long acting and well absorbable cardiac glucoside, digito-xin reduced the ocular tension from 15 to 75 % (average 43%). Neither intoxication nor visual disturbances including visual field were observed.
