頸動脈ステント留置術が視機能改善に有効であった眼虚血症候群の1例

Improved visual function following stenting of carotid artery in a case of ocular ischemic syndrome

要約　目的：視力低下を契機として内頸動脈狭窄が発見され，ステント留置術後に視機能が改善した症例の報告。症例：76歳女性が2か月前に突発した左眼視力低下で受診した。高血圧があり，47年前からの喫煙歴がある。所見：矯正視力は右0.9，左0.5で，眼圧は左右眼とも10mmHgであった。左眼に虹彩ルベオーシスとテント状の周辺虹彩前癒着（PAS）があった。眼底に格別の異常はなかった。超音波検査で，左内頸動脈に75％の狭窄があった。頸動脈ステント留置術を行い，その2日後に左眼視力は0.8になり，虹彩ルベオーシスはほとんど退縮し，術前にあった鼻側の視野沈下は改善した。結論：内頸動脈狭窄に対するステント留置術で，視機能が改善した。

Abstract.　Purpose：To report a case of ocular ischemic syndrome that improved after stenting of the internal carotid. Case：A 76-year-old female presented with impaired vision in the left eye since 2 months before. Findings：Corrected visual acuity was 0.9 right and 0.5 left. Intraocular pressure was 10mmHg in either eye. The left eye showed rubeosis iridis and peripheral anterior synechia. Fundus finding was unremarkable. Echography showed stenosis of left internal carotid by 75％. Two days after stenting of the left carotid artery, visual acuity improved to 0.8, rubeosis iridis almost disappeared, and depressed isopter in the nasal sector improved. Conclusion：Visual function improved following stenting of the carotid in a case of ocular ischemic syndrome.
