樹氷状網膜血管炎を呈したヒトT細胞型白血病ウイルス陽性の1例

Frosted retinal branch angiitis in a case positive for human T-cell leukemia virus

要約　目的：樹氷状血管炎の症例の報告。症例：66歳女性が右眼の霧視で受診した。所見：矯正視力は右0.4，左0.7で，眼圧，前眼部，中間透光体に異常所見はなかった。右眼に網膜の樹氷状血管炎があった。光干渉断層計（OCT）で，血管炎に一致して網膜の内層が高輝度であった。血液検査でヒトT細胞型白血病ウイルスが陽性であり，熊本県の出身であることから，ヒトT細胞型白血病ウイルスによるぶどう膜炎と診断した。リンデロン^®点眼で血管炎は消失した。初診から4か月後に左眼にも同様の病変が発症した。結論：樹氷状血管炎の原因として，ヒトT細胞型白血病ウイルスが関与する可能性がある。

Abstract.　Purpose：To report a case of frosted branch angiitis. Case：A 66-year-old female sought medical advice for blurring in her right eye. Findings：Corrected visual acuity was 0.4 right and 0.7 left. The right fundus showed frosted branch angiitis. Optical coherence tomography（OCT）showed high-intensity signal in the inner layer of the retina in areas of angiitis. She was found positive for human T-cell leukemia virus. The home town of the patient was Kumamoto Prefecture in Kyushu. These findings led to the diagnosis of uveitis related to human T-cell leukemia virus. Topical instillation of betamethasone resulted in normalization of fundus findings. She developed another episode of similar fundus lesion in the left eye 4 months later. Conclusion：This case illustrates that frosted branch angiitis may be related with infection by T-cell leukemia virus.
