レーザースペックルフローグラフィを用いた光線力学療法後の血流解析

Choroidal blood flow following photodynamic therapy measured by laser speckle flowgraphy

要約　目的：ポリープ状脈絡膜血管症を光線力学療法で治療した後の脈絡膜血流の変化の報告。対象と方法：インドシアニングリーン蛍光眼底造影でポリープ状脈絡膜血管症と診断された6例6眼を検索した。全例が男性で，年齢は62～83歳（平均74歳）である。レーザースペックルフローグラフィで眼底の病変部とその他の照射部での脈絡膜血流を，照射前と1週間後に測定した。結果：光線力学療法の1週間後の脈絡膜血流は，病変部では平均44.3±33.1％，その他の照射部では40.2±25.5％減少し，いずれも有意であった（p＝0.028）。結論：ポリープ状脈絡膜血管症に対する光線力学療法の1週間後に，病変部とその他の照射部での脈絡膜血流が有意に減少した。

Abstract.　Purpose：To report changes in the choroidal blood flow following photodynamic therapy for polypoidal choroidal vasculopathy（PCV）. Cases and Method：This study was made on 6 eyes of 6 males who were diagnosed with PCV based on findings by indocyanine green angiography. The age ranged from 62 to 83 years,average 74 years. Choroidal blood flow was measured by laser speckle flowgraphy before and one week after photodynamic therapy. Results：Choroidal blood flow decreased by 44.3±33.1％ at the site of PCV and by 40.2±25.5％ in the unaffected but irradiated area after treatment. The decreases were significant（p＝0.028,p＝0.028）. Conclusion：Choroidal blood decreased significantly in the treated area including the site of PCV and its surroundings one week after photodynamic therapy.
