生体吸収性骨固定プレートを用いたKrönlein手術の1例

要約　背景：Krönlein法では眼窩の側壁を切開して骨窓を作り，眼窩内腫瘍などを摘出したのち骨片を金属線などで固定する。目的：骨片の代わりに生体吸収性プレートを用いた症例を報告する。症例：80歳男性が左眼窩部腫脹で受診した。13年前に右涙腺腫瘍摘出を受け，悪性リンパ腫であった。所見：左眼に眼球突出と，眼窩の上外側に腫瘤があった。悪性リンパ腫を疑い，眼窩外側壁切開によるKrönlein手術を行った。あらかじめ切開部の骨形状を記憶させ，生体吸収性ポリマー板を加熱整形し，ネジで固定した。病理組織診断は悪性リンパ腫であり，2年後の現在まで炎症反応や感染がなく，経過は良好である。結論：Krönlein法により眼窩内悪性リンパ腫を摘出し，眼窩側壁の欠損部を生体吸収性プレートで置き換え，良好な結果を得た。

Abstract.　Background：Lateral orbitotomy,or Krönlein surgery,needs closure of the lateral orbital defect using a piece of homologous bone. Purpose：To report a case of orbital tumor treated by Krönlein surgery using a bioabsorbable plate. Case：An 80-year-old male presented with swelling of left orbit. He had received excision of right lacrimal gland tumor 13 years before that proved to be malignant lymphoma. Findings：The left eye showed proptosis with a tumor upper temporal to the orbit. He was treated by lateral orbitotomy,removal of orbital tumor,and closure of bone defect by bioabsorbable polymer plate. The removed tumor proved to be malignant lymphoma. He has been doing well without inflammation or infection for 2 years until present. Conclusion：Krönlein surgery using a bioabsorbable plate was safe and effective in a case of malignant lymphoma of the orbit.
