一過性光視症の主訴により発見された内頸動脈狭窄を伴う小脳橋角部腫瘍

Transient photopsia as the initial manifestation in a case of cerebellopontine angle tumor compressing the internal carotid artery

要約　目的：一過性黒内障を契機として小脳橋角部腫瘍とこれによる内頸動脈圧迫が発見された症例の報告。症例：56歳女性が1週間前にあった左眼の光視症で受診した。7年前に左側の突発性難聴があり加療中である。矯正視力は右0.9，左1.0であり，眼科的に格別の異常はなかった。磁気共鳴画像検査（MRI）などで左内頸動脈の錐体部に強い狭窄があり，ガドリニウム造影で左小脳橋角部に腫瘍が発見された。摘出手術の結果，聴神経鞘腫であった。結論：腫瘍による内頸動脈圧迫で光視症が起こることがあり注意が望まれる。

Abstract.　Purpose：To report a case of cerebellopontine angle tumor and compression of internal carotid artery with transient photopsia as the initial manifestation. Case and Findings：A 56-year-old woman presented with an episode of photopsia in her left eye one week before. She had a sudden attack of hearing loss in her left eye 7 years before. Her corrected visual acuity was 0.9 right and 1.0 left. She showed no pathological ophthalmological findings. Magnetic resonance imaging（MRI）showed stenosis of left internal carotid artery. Gadolinium angiography led to the detection of ipsilateral cerebellopontine angle tumor. It was diagnosed with neurilemmoma of acoustic nerve after surgery. Conclusion：This case illustrates that photopsia may be due to compressed internal carotid artery by intracranial tumor.
