Abstract. Purpose:To report a case of giant lacrimal stone in the canaliculus due to Actinomyces sp. Case:A 71-year-old woman presented with chronic conjunctivitis in her left eye of 2 years' duration. She had an induration inferior to the inner canthus,hyperemia in the palpebral conjunctiva,and hyperemic lacrimal puncta. Hemorrhagic pus flowed out of the punctum by pressure to the induration. Bacterial culture gave negative results. Magnetic resonance imaging(MRI)showed a ring-shaped lesion with very low signal inside on T2-weighted sequences,suggesting actinomycotic cast-forming canaliculitis. Because of its giant size and hard cast,it was removed by transcutaneous approach. The canaliculum was dilated and was filled by green-yellow material. Pathogical studies led to the diagnosis of lacrimal stone due to Actinomyces sp. Conclusion:Giant stone in the canaliculum is to be removed transcutaneously instead of expression or curettage.
