チン小帯脆弱症例の超音波白内障手術におけるカプセルエキスパンダーの試作

Prototype of a capsule expander for phacoemulsification-aspiration in eyes with brittle zonule

目的：チン小帯脆弱眼の超音波白内障手術用に試作した水晶体囊拡張器（カプセルエキスパンダー）の評価。対象と方法：Marfan症候群，落屑症候群，強度近視，外傷，原因不明によるチン小帯脆弱眼各2眼の計10眼を対象とした。年齢は23～81歳，平均59歳である。術後4～10か月，平均6.7か月の経過を観察した。連続円形破囊（CCC）の後，チン小帯断裂の範囲に応じ1～6か所に囊拡張器を設置し，通常の方法で超音波白内障手術を行った。結果：囊拡張器を使用することで，全例で水晶体囊を温存して超音波白内障手術ができた。術中，術後の合併症はなかった。結論：チン小帯脆弱眼の超音波白内障手術に囊拡張器は有用であった。

Purpose：To evaluate a newly designed capsule expander for phacoemulsification-aspiration（PEA）in eyes with brittle zonule. Cases and Methods：Cataract surgery was performed on 10 eyes with brittle zonule. The series comprised 2 eyes each of Marfan syndrome,pseudoexfoliation,high myopia,trauma and unidentified cause. Their ages ranged from 23 to 81 years,average 59 years. They were followed up for 4 to 10months,average 6.7 months,after surgery. Following continuous curvilinear capsulorrhexis（CCC）,one to 6 capsule expander were placed depending on the location on the location of defective zonule. PEA was performed after the standard procedure. Results：Use of capsule expander facilitated PEA by leaving the residual lens capsule intact. There were no intra-or postoperative complications. Conclusion：Capsule expander was useful for PEA in eyes with brittle zonule.
